Our dental insurance is provided by Delta Dental of Tennessee. The dental plan under Delta Dental offers a broad network of providers to use to obtain benefits. In fact, Delta Dental offers two networks together as part of our plan.

The Delta Premier network is their largest network of providers and provides discounted rates to you. The Delta PPO network is about half the size of the Premier network, but provides greater discounted rates for you. You have the choice at the time of service of which network to use. You may also use out-of-network providers but the plan benefits are reduced and you do not receive discounts.

When using a Delta Dental in-network dentist:

Preventive Services are covered at 100% with no deductible;
Basic Services are covered at 80% with a $50 deductible;
Major Services are covered at 50% with the $50 deductible;
Orthodontic Services are covered at 50%, with a separate $50 deductible and a maximum lifetime benefit of $1,500, per covered individual
Maximum yearly benefit is $1,500.

You do not have to use a Delta network dentist; however, your out-of-pocket costs will be less in-network.

Out-of-network services are reimbursed at usual, customary and reasonable (UCR) rates based on the 90th percentile of dentists in the area, reducing the out-of-pocket expense for employees.

Dental Insurance premiums can be paid under the Cafeteria plan and paid with pre-tax dollars.
