Critical Illness coverage will end with Allstate 6/30/2025
Starting 7/1/2025, Critical Illness will include Cancer and be through Voya.
The plan pays a lump sum payment for diagnosis of a covered Critical Illness and adds Cancer coverage as a Critical Illness.
There is no total maximum benefit amount or limit to the number of payments you may receive for each covered condition under your plan, except for skin
cancer (1 x year 10% for 10 years)
Allstate Rates
Rates are based on the Employee age when applied for and amount of coverage elected.
Spouse can be elected for 50% of the Employee Amount.
Child/ren are covered at 50% for no additional cost

Guaranteed Issue, as a new hire or first eligible. Meaning no medical questions to answer when you are first eligible under Allstate.
Voya has No Pre-existing Conditions, No Waiting Periods and is Guarantee Issue each Annual Enrollment Period.

- Employee: $10,000 to $20,000
- Spouse- 50% of employee: $5,000 to $10,000
- Children- 50% of employee Birth to 26: $5,000 to $10,000

Older Critical Illness Policy 2012 to 2019
Benefits will be reduced by 50% on the certificate anniversary following the insured’s 70th birthday. The premiums will not decrease.
