Employees can elect the AllState Cancer Plan that covers Cancer and 29 other Dreaded Diseases. Employees have the option of selecting from two benefit plans. The plan is insured by AllState
Guaranteed Issue –No Medical Questions to answer at initial enrollment.
It will not pay benefits if there is a pre-existing condition during the 12 months prior to the effective date of the new policy.

In addition to Cancer, the plan covers Muscular Dystrophy, Amyotrophic Lateral Sclerosis (Lou Gehrig’s Disease), Poliomyelitis, Multiple Sclerosis, Encephalitis, Rabies, Tetanus, Tuberculosis, Osteomyelitis, Diphtheria, Scarlet Fever, Cerebrospinal Meningitis (bacterial), Brucellosis, Sickle Cell Anemia, Thalassemia, Rocky Mountain Spotted Fever, Legionnaire’s Disease (confirmation by culture or sputum), Addison’s Disease, Hansen’s Disease, Tularemia, Hepatitis (B or C with liver failure or Hepatoma), Typhoid Fever, Myasthenia Gravis, Reye’s Syndrome, Primary Sclerosing Cholangitis (Walter Payton’s Liver Disease), Lyme Disease, Systemic Lupus Erythematosus, Cystic Fibrosis, Primary Biliary Cirrhosis.

Premiums are paid on a “Pre-Tax” Basis.
Employees can enroll dependent children to age 26 regardless of student status!
Plan can be converted to an individual policy and continued when employees leave/retire from Putnam County Schools.
