Each employee has the option of selecting additional Group Life insurance on their Dependent Spouse in increments of $5,000 up to $100,000. Amount selected for spouse cannot exceed amount of supplemental group life insurance selected for employee.
A Dependent Spouse’s amount of Optional Life Insurance terminates when the Dependent Spouse attains age 70.

Evidence of Insurability
If the spouse enrolls within 31 days of your eligibility date, coverage is issued on a guarantee issue basis (limited to $25,000 for spouse). Amount selected cannot exceed 100% of employee Optional Life amount.
If you wish to increase spouse coverage above $25,000, evidence of insurability, acceptable to the insurance carrier must be submitted.
