Benefits At a Glance
Langdale now offers a new group Short Term Disability plan for all eligible employees.
Employees not eligible for the company LTD can elect to enroll in a short-term disability plan that provides up to 60% of pre-disability earnings.
Maternity benefits are paid the same as for an illness. Benefits are not paid for occupational illness or accident. Payroll deduction costs are based on earnings and age as of January 1st of each year.
No EOI is required, but a 12/12 Pre-X is required, meaning anything that you have been treated for in the past 12 months will not be eligible for the benefit for the upcoming 12 months.
Group Short Term Disability Insurance
Short-term Disability Insurance replaces a portion of your income when you are recovering from a covered illness or injury.
Two plan options are available.
Option 1
- Elect any $50 increment, with a minimum of $100 of weekly coverage, up to $1,500 per week, not to exceed 60% of salary
- Benefits begin on the 15th day and last 13 weeks
Option 2
- Elect any $50 increment, with a minimum of $100 of weekly coverage, up to $1,500 per week, not to exceed 60% of salary
- Benefits begin on the 31st day and last 26 weeks
Plan Design
Pre-Existing Condition
- Any sickness or injury for which you have received medical treatment, consultation, care, or services (including diagnostic measures or the taking of prescribed medications) during the specified months prior to your coverage effective date.
- A disability arising from any such sickness or injury will be covered only if it begins after you have performed your regular occupation on a full-time basis for the specified months following the coverage effective date.
Benefit Exclusions
You will not receive benefits in the following circumstances:
- Your disability is the result of a self-inflicted injury.
- You are not under the regular care of a doctor when requesting disability benefits.
- Your disability is the result of war, declared or undeclared, or any act of war.
- Your disability is covered under a worker’s compensation plan and/or is due to a job-related sickness or injury.
