Employee Optional Life
Each employee is eligible to select from $10,000 up to $500,000 additional group life insurance coverage, ( in $10,000 increments).
Monthly payroll deduction costs are based on age as of July 1st, per the below table:

Spouse Optional Life
Each employee is eligible to select an amount of coverage on their spouse equal to the lesser of 50% of the amount of employee optional life selected or $100,000.
Selection must be in increments of $5,000.

Additional Features
Accelerated Death Benefit which allows you to receive a portion of your life insurance in the event of a terminal illness with a 12 month life expectancy.
Portability allows employees to continue optional life insurance protection for themselves, spouse, or dependents when changing to a new employer. Offered at group rate, this is an affordable way to continue life insurance coverage. No medical exams are required and the employee may elect continued coverage of 100%, 75% or 50% of current life insurance coverage, subject to a maximum of $250,000. Continued coverage of spouse or dependent children is subject to maximum amounts of $50,000 and $10,000 respectively. This Portability option is available to terminating employees who have not reached Normal Retirement Age as defined by the 1983 Federal Social Security Act (generally age 65 or later). Coverage may continue to age 75 with a reduction age 65 to 25% of the original amount.

Waiver of Premium– In case of permanent and total disability, premiums are waived and coverage is continued to age 65 or Normal Social Security Retirement Age.
A “waiver of premium” claim must be filed within one year from the last day worked

Enrollment
Guarantee Issue and Evidence of Insurability
If applied when first eligible as a New Hire, 3 times their annual earnings or a maximum of $150,000 is available as Guarantee Issue for Employee Group Life! Up to $20,000 is available for spouse life, without providing evidence of insurability.
Employees may enroll or increase $10,000 each annual enrollment up to the Guarantee Issue Limits. Spouse coverage can be elected or increased $5,000 each annual enrollment up to Guarantee Issue Limits. Additional coverage amounts will require evidence of insurability that is satisfactory to The Hartford before the excess can become effective.
